Breakup, Pain and Rejection: The Psychology Behind Why It Actually Hurts

You know the feeling. Someone you loved stops loving you back, and suddenly there's this dull ache sitting right in your chest. You clutch a cushion, you can't eat, you keep saying "it hurts" as if you've actually been injured. Not sad. Not disappointed. Hurt.


Psychology says that the brain does not differentiate between physical and emotional pain. Scientific research has been conducted where fMRI scans were studied of people who went through emotional pain, and what they found is a little wild: heartbreak and physical pain aren't just similar-sounding phrases. Deep in your brain, they're genuinely tangled up with each other.

Why Do Some People Just Feel Everything More?


Pain sensitivity and emotional sensitivity are not the same in every brain. Some regions involved in registering distress can be underdeveloped, poorly regulated, or simply wired to fire more easily than others.


Take the amygdala. Its job is to rapidly detect emotionally significant information, a threat, a loss, a rejection, and amplify it. Then there is the prefrontal cortex, which acts as the brain's executive manager for emotion. It puts feelings into perspective, inhibits excessive emotional reactions, and plans a measured response instead of letting you react on impulse. Stronger connectivity between the prefrontal cortex and the amygdala allows for better regulation, while weaker connectivity means the amygdala's alarm keeps ringing largely unchecked, so the emotion stays intense for longer.


There is a chemical piece to this too. Lower serotonergic activity is associated with greater emotional reactivity, which is part of why some people ride out heartbreak fairly steadily while others feel every wave of it at full volume.


None of this is a flaw in character. It is simply a difference in wiring, and wiring like this can shift and strengthen over time.



Your Brain Doesn't Fully Know the Difference


In a well-known 2011 study done by neuroscientist Ethan Kross and his team, they recruited people who had recently gone through an unwanted breakup and put them in an fMRI scanner. Each person did two things while their brain was being imaged. First, they looked at a photo of their ex while thinking about the breakup. Later, the same participants had a genuinely painful hot sensation applied to their arm.


When people viewed the photo of their ex, the scans lit up in the secondary somatosensory cortex and the dorsal posterior insula, regions that are not just linked to sadness in general, but to the physical, sensory experience of pain itself. When the same people were exposed to the hot stimulus, those same regions activated again, in a strikingly similar pattern.


This built on earlier work by Eisenberger (2003), who found that social exclusion activates the anterior cingulate cortex and anterior insula, areas long associated with the emotional distress side of pain processing. Alongside this activity, the brain also releases endorphins, the same chemicals your body uses to dull physical injury. Put simply, your brain does not have a separate filing system for "heart hurts" and "body hurts." It has one pain system, and rejection walks straight into it.


So What Do You Actually Do With This?


Since heartbreak triggers the same circuitry as an actual injury, it makes sense to treat it like one.


  • Speak about it: Telling a friend "this hurts" is not an exaggeration; it is accurate. Naming the pain out loud has been shown to ease emotional strain rather than dramatise it.

  • Help the body, not just the mind: Because physical and emotional pain share wiring, the things that soothe physical pain, warmth, touch, rest, genuinely help here too.

  • Don't isolate yourself: Withdrawing feels natural after rejection, but research consistently shows that social support lowers the intensity of pain, both physical and emotional.

  • Consider therapy: A therapist can help you process what happened, build healthier coping tools, and learn emotional regulation, rather than just waiting for time to do the work alone.

  • Remember the brain can change: Neuroplasticity means the pathways connecting the amygdala and the prefrontal cortex are not fixed for life. With consistent practice, whether through therapy, mindfulness, or simply repeatedly choosing a regulated response over a reactive one, that connectivity can strengthen, and emotional reactions that once felt overwhelming start to settle faster.



Keep an Eye Out


This is also part of why some people turn to self-harm (cutting, slapping, punching walls) after a painful breakup or rejection. If someone's brain has learned that physical pain brings a rush of numbing endorphins, it can become a way of coping with emotional pain that feels unbearable. This is not a character flaw; it is the same overlapping pain system working against them. If you notice this in someone you love, the most useful thing you can do is help them access proper therapy and treatment, not judgment.
